Personal Hygiene: Healthy Habits Include Good Grooming

If you want to minimize your risk of infection and also enhance your overall health, follow these basic personal hygiene habits:

  • Bathe regularly. Wash your body and your hair often. “I’m not saying that you need to shower or bathe every day,” remarks Dr. Novey. “But you should clean your body and shampoo your hair at regular intervals that work for you.” Your body is constantly shedding skin. Novey explains, “That skin needs to come off. Otherwise, it will cake up and can cause illnesses.”
  • Trim your nails. Keeping your finger and toenails trimmed and in good shape will prevent problems such as hang nails and infected nail beds. Feet that are clean and dry are less likely to contract athlete’s foot, Novey says.
  • Brush and floss. Ideally, you should brush your teeth after every meal. At the very least, brush your teeth twice a day and floss daily. Brushing minimizes the accumulation of bacteria in your mouth, which can cause tooth decay and gum disease, Novey says. Flossing, too, helps maintain strong, healthy gums. “The bacteria that builds up and causes gum disease can go straight to the heart and cause very serious valve problems,” Novey explains. Unhealthy gums also can cause your teeth to loosen, which makes it difficult to chew and to eat properly, he adds. To maintain a healthy smile, visit the dentist at six-month intervals for checkups and cleanings.
  • Wash your hands. Washing your hands before preparing or eating food, after going to the bathroom, after coughing or sneezing, and after handling garbage, goes a long way toward preventing the spread of bacteria and viruses. Keep a hygiene product, like an alcohol-based sanitizing gel, handy for when soap and water isn’t available.
  • Sleep tight. Get plenty of rest — 8 to 10 hours a night — so that you are refreshed and are ready to take on the day every morning. Lack of sleep can leave you feeling run down and can compromise your body’s natural defenses, your immune system, Novey says.

Personal Hygiene: Poor Hygiene Hints at Other Issues

If someone you know hasn’t bathed or appears unkempt, it could be a sign that he or she is depressed. “When people are sad or depressed, they neglect themselves,” Novey says. Talking about the importance of proper personal hygiene for preventing illnesses and providing personal hygiene items may help some people. Be candid but sensitive and understanding in your discussions, Novey says. Despite your best efforts, your friend or loved one may need professional help. You should encourage them to see a counselor or doctor if their personal hygiene doesn’t improve.

Shopping for Home Care: Information You Need to Know

Who Is Eligible?

shopping for home careHome care comes in a variety of offerings from government subsidized to free market, privately paid services. Anyone is eligible for home care services for which one pays a fee, sometimes called private duty home care or private pay home care. A professional home care agency will come into the home first, before services are started, to perform an assessment and help determine the right amount of care for that person’s specific needs. A personal plan of care is standard and is also the result of that first step when assessment for care is completed. Although most people prefer to remain in their own homes, it may be determined that care in a facility is more appropriate for the particular situation. This will depend on the physical and mental capacities of the person needing care, the support system in place to provide ancillary services, and the person’s ability to pay for services.

Who Pays for Care?

There are many different sources to help pay for care, including federal veteran’s benefits, Medicaid, some long-term care insurance policies, worker’s compensation and some local disease support and advocacy organizations. Eligibility does come into play if you are looking for help paying for care from any source other than private funds. Elder law attorneys and estate planners can be very helpful when planning for the payment of long-term care.

What About Medicare?

It is a common misconception that Medicare will pay for long-term care needs, when in fact the opposite is true. Medicare is health insurance coverage provided by the United States government for people over age 65 that covers skilled medical care; it does not customarily cover assistance with daily living activities such as bathing, dressing and grooming. Some benefits may be available for short-term home health care, but only if certain conditions are met.

Registry/Private Hire vs. the Home Care Agency

Although hiring a caregiver privately or through a registry may seem more affordable than a professional agency at first glance, there are hidden costs and risks associated with the private hire and registry.

  • Tax Issues/Worker’s Compensation: A professional agency employs its caregivers and is responsible for all worker’s compensation and Social Security, unemployment and payroll taxes for its employees. When hiring privately or through a registry, you become the employer and are required to pay for all of those items. Additionally, with the private hire or registry model, you would be held liable for any work-related injury that occurs on the job. This can include the cost of all medical expenses and any disability payments that might become applicable.
    Even if the caregiver represents himself or herself as an independent contractor, it is your responsibility to make sure this is actually the case, as oftentimes the caregiver will not meet the legal criteria as an independent contractor, in which case the tax and worker’s compensation responsibilities will be yours.
  • Supervision: Unmonitored, unsupervised care means a greater chance that care providers can exert undue influence on a frail person for personal financial gain, or the possibility of emotional or physical abuse. A professional agency will provide routine supervision and care monitoring.

To help determine if a company is a registry or a professional agency, be sure to ask the company if their caregivers are employed by the company, if they cover worker’s compensation for their caregivers and if they monitor and supervise their caregivers.

Assessing Needs

With a professional home care agency, you don’t need to worry about determining what services are best for a particular situation. Before the start of care, the agency will come into the home and ask several questions in order to come up with a care plan that is personalized to the situation and suits the specific needs of the person needing care. The agency will also work with the client’s physician, or others in the care team, and help coordinate other care services as needed.

Who Manages the Care?

A professional home care agency will have a supervisor, oftentimes a nurse, who will perform regular check-ins and update the care plan as needs change. Many clients also work with a professional geriatric care manager to assist with coordinating care and serving as the client’s advocate.

Care Transitions

Care needs can change from basic assistance with daily activities and personal care to skilled healthcare to rehabilitative care and back to basic assistance. Keeping track of everything you eat and drink in a day is a tedious task that is tough to keep up with over time. Unfortunately, dutiful tracking is a vital component for successful weight loss, however, a new study in Obesity finds that perfect tracking is not needed to achieve significant weight loss.

Researchers from UConn, the University of Florida, and the University of Pennsylvania tracked 153 weight loss program participants for six months where users self-reported their food intake using a commercial digital weight loss program. The researchers wanted to see what the optimal thresholds were for diet tracking to predict 3%, 5%, and 10% weight loss after six months.

“We partnered with WeightWatchers, who was planning on releasing a new Personal Points program, and they wanted to get empirical data via our clinical trial,” says co-author and Department of Allied Health Sciences Professor Sherry Pagoto.

Pagoto explains that the new program takes a personalized approach to assigning points including a list of zero-point foods to eliminate the need for calculating calories for everything,

“Dietary tracking is a cornerstone of all weight loss interventions, and it tends to be the biggest predictor of outcomes. This program lowers the burden of that task by allowing zero-point foods, which do not need to be tracked.”

Researchers and developers are seeking ways to make the tracking process less burdensome, because as Pagoto says, for a lot of programs, users may feel like they need to count calories for the rest of their lives: “That’s just not sustainable. Do users need to track everything every single day or not necessarily?”

With six months of data, Assistant Professor in the Department of Allied Health Sciences Ran Xu was interested to see if there was a way to predict outcomes based on how much diet tracking participants did. Ran Xu and Allied Health Sciences Ph.D. student Richard Bannor analyzed the data to see if there were patterns associated with weight loss success from a data science perspective. Using a method called receiver operating characteristics (ROC) curve analysis they found how many days people need to track their food to reach clinically significant weight loss.

Getting regular medical care
Most health plans give you the best deal on services when you see a doctor who has a contract with your health plan. While you may be able to see doctors who don’t contract with your plan, visiting an “in-network” provider usually means you’ll have lower out-of-pocket costs.

Finding a doctor in your plan

To find out if your doctors and other health care providers are covered by your new Marketplace plan, or to find a covered provider if you don’t have one yet:
  1. Visit your health plan’s website and check their provider directory, which is a list of the doctors, hospitals, and other health care providers that your plan contracts with to provide care.
  2. See your health plan’s provider directory. You can get this by contacting your plan, visiting the plan’s website, or using a link that you’ll find on the plan description in your Marketplace account.
  3. Call your insurer to ask about specific providers. This number is on your insurance card and the insurer’s website.
  4. Call your doctor’s office. They can tell you if they accept your health plan.
  5. Call the Marketplace Call Center at 1-800-318-2596 (TTY: 1-855-889-4325). A trained representative can help you find your insurer’s number

Is there an appeals process if I go to my regular doctor and find out later that my new plan doesn’t cover them?
